Set the organisation's rule for one model lane (idempotent upsert)

Answers 409 MODEL_RULE_LANE_MANAGED when the lane rule is managed by system or iac (it is never overwritten here), 409 MODEL_RULE_LANE_EXISTS when a concurrent writer holds the lane, and 409 MODEL_RULE_LANE_CHANGED when the lane rule was deleted between the read and the update (nothing was written; re-read and retry).
